Massachusetts State Law requires that all incoming students provide verification of specific immunizations. Documentation of immunizations must be received prior to the start of classes.

Your health care provider should complete the Immunization History Form and administer any immunizations that have not yet been received. You should read and return the Meningoccal Information and Waiver From if you are not receiving this immunization.
