Immunization and Wellness Education Requirements

The Boston Conservatory prides itself on being a safe and healthy place for students to study and live. In order the assure the health of students, the Conservatory is committed to complying with Massachusetts state law, which requires specific immunizations for all full-time undergraduate and graduate students, and all students attending any postsecondary institution while on a student or other visa. This includes foreign students attending or visiting classes as part of a formal academic visitation or exchange program. Required immunizations are:

  • Hepatitis B: 3 doses 
  • Tdap: 1 dose within past 10 years 
  • MMR: 2 doses 
  • Varicella: 2 doses 
  • Meningococcal: 1 dose for all full-time students living on campus, undergraduate and graduate

All students must submit a completed Boston Conservatory Immunization History Form which documents their immunization history or shows proof of immunity. The Immunization History Form must be signed or stamped by a physician or licensed medical professional. You must submit our form, filled out in its entirety. Please find the form below. 

Students starting in the Spring 2016 semester must submit their Immunization History Form by January 4th, 2016. 

If you do not submit your documents by the deadline, you will not be able to register for classes.

Translated Immunization History Form for International Students

All incoming students must also complete Think About It, an online program about health and wellness. The due date for incoming Spring 2016 students is January 4th, 2016. Please read about Think About It and complete it by the deadline.
