Immunization and Wellness Education Requirements

The Boston Conservatory prides itself on being a safe and healthy place for students to study and live.  In order the assure the health of students, the Conservatory is committed to complying with Massachusetts state law which requires specific immunizations for all full-time undergraduate and graduate students, and any full-time or part-time students attending any postsecondary institution while on a student or other visa, including foreign students attending or visiting classes as part of a formal academic visitation or exchange program. These immunizations are:

  • Hepatitis B: 3 doses for all full-time undergraduate and graduate students
  • Tdap: 1 dose within past 10 years for all full-time undergraduate and graduate students
  • MMR: 2 doses are required for all full-time undergraduate and graduate students
  • Varicella: 2 doses are required for all full-time undergraduate and graduate students
  • Meningococcal: 1 dose for all full-time residential students, undergraduate and graduate

All required students must submit a completed Boston Conservatory Immunization History Form which delineates their immunization history or shows proof of immunity.  The Immunization History Form must be signed or stamped by a physician or licensed medical professional.

For health and safety reasons, all incoming students must also complete My Student Body training on alcohol and sexual violence prevention before coming to campus.  Please click here to find instructions on how to complete My Student Body.

Until these two requirements have been completed, students will not be able to access on-line Self Service to register for classes, add/drop classes, see their schedules, view their grades, view and pay their bill online, and obtain unofficial and official transcripts. If students do not complete the requirements of the Immunization and Wellness Education Policy, they will continue to have a hold on their account disabling Self Service.

Forms for new students entering in Fall 2014 should be submitted no later than Monday, August 4th.

Please note: additional forms or records WILL NOT BE ACCEPTED. Please do not submit any other documents regarding your health history. Additional documents will not be returned and will be destroyed.
